Please fill in all the fields in this form, incomplete applications will not be processed. Please be as descriptive as you can in the class-related fields. All new classes and professors have to be confirmed by the Governors Board, you will be contacted by Chauncey Clemenceau on March 12, 2007 with either an acceptance or denial. ONE CLASS PER APPLICATION, YOU MAY SUBMIT AS MANY APPLICATIONS ARE YOU WANT.

Your Name:	What name do you want as an Professor? (No canon characters from any book or movie, and no famous people.)
Your Age:	All Professors MUST be over 16 years old.
Email Address:	The E-mail address you want to use for yourself. Must exist already.
Student Character:	Do you, or have you ever had a student character on Hogwarts Online? If yes, what was the name?
Describe Yourself:	Tell us something about yourself; how old are you, what do you do? Why do you think you qualify for the topic you want to work for Hogwarts Online? What special qualities do you have that would be an asset to the Hogwarts Online?
References:	Please list at least three references (you must know these references and they must know you). Each reference must currently be a staff member of the site.
Website:	In order for your application to be accepted you MUST already have a website that contains at least 1 finished lesson and assignment and a course guide. Without a website your application will not be processed.
Course Title:	What is the name of the course that you want to teach?
Course Length:	Select how long your course will run, either a 'Half Year' Course or a 'One Term' Course. This is similar to semesters in High School. If a course is One Term, it will only run for a term, then repeat. If a course is Half Year, it will run for two terms and not repeat again until next year.
Course Outline:	Please give a paragraph description on what you plan to teach in the course, and what type of assignments you would like to have. How will the lessons be set up? What type of assessments will be used? Anything special about this class?
Syllabus:	Please give a detailed listing of the topics that you plan to cover with this course and general time frame when this course will have assignments.
